Stabilization and transitional program for homeless men with a history of substance abuse problems.

How do I refer someone or get admitted to this program?

Transitions clients must be referred via a program-therapist, probation officer, attorney, or a religious leader such as a minister, rabbi, or priest. 
Note: The Transitions Program cannot accept clients on parole or bail nor any clients with a documented history of sexual or arson offences. Referral sources should call the number above and ask to speak with someone from the Admissions Department. You will be faxed a one-page referral form, which, once completed, you will fax back to us with a copy of the client's psychosocial assessment.

All clients must have a current TB screen completed before being admitted to the program. Also, if the potential client is not already in a residential program, a urinalysis result must also be obtained and forwarded.

All information is reviewed and the referral source will be re-contacted within 24 hours with a decision and a date and time of client admission.
